Photography & Video Consent
I grant Gotta Dance Inc. permission to take photographs and/or video recordings of my child for promotional use. These images may be used in digital or print formats, including but not limited to social media, brochures, newsletters, and advertising.
No names or personal details will be shared, and I understand that no compensation will be provided for the use of such media.
Waiver & Assumption of Risk
I understand that dance involves physical activity and may carry the risk of injury. By registering my child for classes at Gotta Dance Inc., I voluntarily assume all risks associated with participation in dance classes, rehearsals, performances, and studio events.
I release and hold harmless Gotta Dance Inc., its owners, instructors, employees, and volunteers from any liability for injury, loss, or damage resulting from participation in any Gotta Dance activity. I understand that it is my responsibility to communicate any medical conditions or concerns to studio staff prior to participation.
In the event of a medical emergency, I authorize Gotta Dance staff to take appropriate action and understand that I will be contacted as soon as possible.
